You can request certified copies of vehicle records by sending an e-mail to vehiclecerts@dot.state.wi.us.

Include the following in your request:

  • Name of the defendant
  • Address (defendant)
  • Owner name (if other than the defendant)
  • Address (owner)
  • Date of violation
  • Alleged violation or criminal charge
  • Vehicle make/year
  • Vehicle identification number
  • Court date
  • Any special instructions
  • Your agency name, contact person, mailing address and phone number

All other certified vehicle record requests

To process your request for a certified copy of a vehicle record, complete a Vehicle/Driver Record Information Request form MV2896 PDF . Indicate in the comment area of Section B that the record needs to be certified.

The certification fee is $10. Make check payable to "Registration Fee Trust." We will bill you if additional fees are required to complete your request.
