Before we can process your request for a certified copy of a driving record, you will need to complete a Vehicle/Driver Record Information Request form MV2896 .

The certification fee is $10. Make check payable to "Registration Fee Trust."

Mail check and form MV2896 to:

Madison, WI 53707-7917

Your request will be processed within 48 hours of receipt.
