Replacement license plates

You can apply for a replacement license plate by downloading and completing a Replacement or Duplicate License Plates Application form MV2118 Adobe PDF icon. This form is also available at any DMV service center. Complete the vehicle description on form MV2118 for the plate that needs replacing and submit the form with the correct fees.

NOTE: Form MV2118 can not be used to change from one type of plate to another. If you need to change the plate type or vehicle weight, you will need to complete the Wisconsin Title & License Plate Application form MV1 Adobe PDF icon (349 KB) and submit it along with the appropriate fee.

Non-personalized and non-personalized special interest plates:

  • Your plate will be replaced with a NEW number.
  • When you receive the new plate(s), destroy the old plate(s). If your plates were stolen, notify your local law enforcement agency of the stolen plate number.
  • You will receive a new Certificate of Registration in 3-4 weeks. The plates are mailed out separately from the registration and may take an additional 2-4 weeks.

Payment

The fees for a replacement plate vary from $2 to $6 for each plate depending on the type of license plate. The fees are shown on the form.

  1. Make check payable to: Registration Fee Trust
  2. Mail the application along with the appropriate fee to:

If you e-mail us about a specific vehicle, we need your license plate number, the year and make of the vehicle and/or the vehicle identification number to respond. WisDOT may need to research your question before replying, please allow 4 business days for an e-mail response. Please do not send a second e-mail.
  • Call: (608) 266-1466, between 7:30 a.m. and 4:30 p.m., Monday through Friday.

Personalized and Special Interest personalized plates:

  • Complete the plate and vehicle description on form MV2118 Adobe PDF icon.
  • You will be issued duplicate plate(s) with the same personalized number.
  • You will receive your new plates in 6-8 weeks.
  • If personalized plates have been reported as stolen, send the application to:
