Form MV2117 is restricted to lienholder use only.

To apply for a title in your (lienholder) name as repossessor

Complete and sign Section A of form MV2117 PDF:

  • Your secured party number, name and address must be the same as the secured party information shown on the Wisconsin Certificate of Title.
  • If the new title is to be mailed to someone other than the secured party name and address listed on the Certificate of Title, complete the "Special Mailing – Name and Address" portion.
  • If more than one lien holder is listed on the title you must notify subsequent lien holders of the repossession and confirm it by checking the "Notice of Sale given subordinate lienholders" box.
  • If you have a purchaser for the vehicle, the purchaser must complete form MV1 PDF (401 KB) -Application for Title/Registration and submit it along with the MV2117 and proper fees.

Section B

Complete section B and sign when the vehicle has been repossessed by someone other then the secured party shown in section A.

Section C

Complete section C and sign when the vehicle has been repossessed outside Wisconsin.

Mail the completed MV2117 PDF form along with the title fee to:

You can receive priority service by including an additional $4 and mailing the completed MV2117 to the priority service address:

You may also take the application to any DMV service center offering title and registration services for immediate processing. There is an additional $5 fee for processing at a DMV service center.
